I once attended a training seminar that left a lasting impression on my approach to design. During the session, the instructor drew a simple illustration of three people standing behind a fence: a child and shorter adult who could not see over it, and a taller adult who could.
The instructor explained that equality means giving everyone the same resources or conditions. Equity, however, means providing each person with what they need to achieve the same outcome. In the illustration, boxes were placed under the child and the shorter adult so that all three people could see clearly over the fence.

That image has stayed with me. It captures something that numbers and policy documents often can’t: the difference between fairness as a formula and fairness as an outcome.
Healthcare organizations often discuss health equity through the lens of policy, funding, and clinical practice. While those factors are essential, health equity is also a design challenge. Every decision, from where a facility is located to how patients navigate a waiting room, influences who can access care, who feels welcome, and who is ultimately able to achieve positive health outcomes.
Research consistently shows that access, environment, communication, and systemic barriers all shape whether patients receive timely, effective care and whether they return.1 The built environment plays a critical role in addressing these gaps.2
The following nine design considerations demonstrate how healthcare environments can become powerful tools for advancing health equity.
1. ACCESS TO CARE
Equitable healthcare begins before a patient even walks through the door. Transportation, proximity, and physical ease of entry are social determinants of health, factors that directly influence whether patients receive timely care.
Design strategies that address access include locating facilities within underserved or rural areas, incorporating Freestanding Emergency Departments (FSEDs) to extend reach, prioritizing walkability and safe pedestrian pathways, and providing connections to public transit and mobility programs
For health systems, these decisions carry real operational weight. Facilities that reduce access barriers consistently see higher utilization rates among vulnerable populations and stronger community health outcomes as a result.
2. TRAUMA-INFORMED DESIGN
Trauma is more common than many people realize. More than 70% of individuals have experienced at least one significant traumatic event in their lifetime.3 Healthcare environments (which are already associated with anxiety and vulnerability for many patients) must account for this reality.
Trauma-informed design centers on psychological safety. Key applications include acoustic materials that absorb or block sound for privacy, reduced auditory stimulation for sensory-sensitive patients, distinct quiet zones separate from active waiting areas, legible and predictable spatial layouts, layered or dimmable lighting, and clear sightlines to exits so patients feel a sense of control rather than confinement.
Environments that communicate safety, trust, and predictability improve patient engagement and can meaningfully reduce the barriers that keep trauma survivors from seeking care in the first place.
3. STRESS REDUCTION THROUGH WAYFINDING
Navigating an unfamiliar healthcare facility is stressful under the best circumstances. For patients with physical limitations, cognitive differences, or limited English proficiency, a confusing environment can create delays in care or deter future visits entirely.
Effective wayfinding design treats navigation as a clinical concern. Multilingual signage and icon-based systems reduce language barriers. Color zoning and intuitive spatial sequencing lower cognitive load. Visible information desks at entry points and consistent, accessible pathways give patients confidence from the moment they arrive.
Studies show that patients who can navigate clearly report higher satisfaction and greater trust in their care experience, linking design decisions directly to health outcomes.4
4. MOBILITY INCLUSIVITY
A healthcare facility must serve patients across a full spectrum of physical and cognitive ability and that spectrum is broader than most standard accessibility checklists acknowledge. Universal Design goes beyond code compliance to create spaces that are genuinely usable for everyone, regardless of mobility level, age, or disability.
Practical applications include adjustable toilet and sink heights, automated doors and lighting systems, wider corridors and door clearances, and reduced travel distances between entry points and care spaces. These elements improve safety and independence for patients and ease of movement for staff as well.
When accessibility is designed rather than retrofitted, it tends to serve everyone better.
5. PHYSICAL COMFORT
Comfort is a fundamental component of equitable care. Patients who feel physically supported are more likely to engage positively in their care experience, communicate openly with providers, and return for follow-up.
Design strategies include adjustable exam tables that accommodate varied mobility levels, bariatric-inclusive seating that is integrated and non-stigmatizing, seating with subtle motion for neurodiverse users, and higher seating options for patients with joint or mobility challenges.
The goal is to create an environment where patients feel comfortable and at ease when waiting to be seen.
6. RESTORATIVE DESIGN AND BIOPHILIA
Evidence-based design research has established a clear relationship between the physical environment and patient outcomes. Access to natural light, exterior views, and nature-inspired elements has been shown to reduce stress, support recovery, and improve overall well-being.
For healthcare facilities serving high-stress or underserved populations, these design choices carry particular weight. Key elements include maximizing access to daylight and outdoor views, incorporating nature-inspired artwork and materials, integrating locally resonant imagery and cultural touchstones, using warm organic finishes, and applying biomorphic geometry and fractal patterns shown to have calming neurological effects.
Restorative environments do more than feel pleasant. They actively support the physiological conditions under which healing occurs.
7. SAFETY AND DURABILITY
Equitable environments must be safe in ways that go beyond physical security. Material choices, such as finishes, adhesives, and furnishings have direct implications for indoor air quality and long-term health.
Low-VOC and non-toxic materials reduce harmful emissions. Durable, easy-to-clean finishes support infection control and longevity. These considerations are especially important in facilities serving underserved communities, where patients are often disproportionately exposed to environmental health risks outside the clinical setting.
The healthcare environment should not add to that burden.
8. TECHNOLOGICAL EQUITY
Technology can bridge healthcare disparities, or it can widen them. Not all patients have equal access to digital tools, equal comfort using them, or equal broadband connectivity at home.
Design strategies that support technological equity include providing both self-service and staff-assisted check-in options, building telehealth-ready spaces into the facility program, and maintaining non-digital alternatives for patients with limited technology access. The goal is not to resist innovation but to ensure that it doesn’t inadvertently create a two-tiered experience.
Inclusive technology integration is a design decision that health systems should make intentionally rather than by default.
9. FLEXIBILITY AND ADAPTABILITY
Patient populations shift. Care models evolve. A facility designed for today’s demographics and delivery methods may be misaligned with tomorrow’s needs within a decade.
Flexible design builds resilience into the program from the start. Modular rooms that support both exam and consultation functions, micro-environments within larger waiting spaces, and the structural integration of behavioral health services alongside clinical care all allow facilities to adapt without wholesale renovation.
For health systems making long-horizon capital investments, adaptability is a financial and operational imperative.
PUTTING IT TOGETHER
From site selection to seat height, designing for health equity requires intention at every scale. The principles above are not a checklist to be completed and set aside. They are a framework for asking, at each decision point: who might this leave out, and what would it take to include them?
Ultimately, equitable healthcare design is about meeting patients where they are—physically, emotionally, and culturally. By aligning design decisions with research, community needs, and inclusive principles, the built environment becomes a powerful tool for reducing disparities and improving outcomes.
